The situation

What has happened

It is 12 September 2026 and you are allocated to rigid haul truck HT22 on day shift, hauling from the north wall to the east dump because the north dump is closed by a tension crack. A high potential vehicle interaction on ramp 4 five days ago has put positive communication at the top of every prestart. HT19 has an intermittent reversing camera and is listed as defect, operable. The shift supervisor expects your prestart inspection recorded in the Mobile Plant and Prestart Register, a Take 5 for the ramp 4 haul, and any defect or hazard reported before you move the truck.

Your brief. Complete the shift prestart meeting requirements, the prestart inspection of HT22, the Take 5 for the haul route and the positive communication calls, record everything in the systems, and report a defect and a hazard correctly during the shift.

What to look for

Evidence guide

Look for a prestart record that follows the procedure item by item, a Take 5 that names the specific hazards of ramp 4 and the closed dump, correct positive communication wording, and a reversing alarm failure treated as a safety critical defect with the truck tagged out. The hazard entry must be factual and rated sensibly.

The student's evidence summary lists every record they created or changed in the systems named above, their notes and the tasks they ticked. Verify it against the deliverables and your own assessment tool.
