Principal hazard
A hazard at a mine or quarry that has the potential to cause multiple fatalities or a single fatality, such as vehicle interaction, ground failure or explosives, and that must have its own management plan.
Take 5
A short personal risk assessment done by the worker before every task: stop, look, assess, control, proceed.
Job hazard analysis
A written, step-by-step risk assessment of a non-routine or high risk task prepared by the crew and approved by the supervisor.
Permit to work
A formal written authorisation to carry out a specific task on plant or in a hazardous area under defined isolations and controls.
Isolation
The separation of plant from every energy source, locked and tagged by each worker and tested before work starts.
Positive communication
A radio call from one vehicle to another that is acknowledged before the vehicles approach, required on haul roads and near heavy plant.
High potential incident
An event that could have caused a fatality or permanent disability, whether or not anyone was hurt, which must be reported to the regulator.
Exclusion zone
An area from which all people and plant are excluded during a hazardous activity such as a blast, a lift or rod tripping.
Trigger action response plan
A table that sets the environmental or ground conditions at which specific actions must be taken, such as wind speeds for dust control or crack widths for dump closure.
Verification of competency
A site-based assessment that a worker can operate a specific machine or do a specific task to the site's standard before being authorised.
