The situation

What has happened

On 7 September 2026 a light vehicle entered the ramp 4 haul road at the Deepridge open cut without positive communication and came within 20 metres of a loaded rigid haul truck on the descent. The truck operator stopped and there was no contact. It is a high potential incident under the vehicle interaction principal hazard management plan and the regulator has been notified. The scene data has been downloaded. The light vehicle driver was a contractor surveyor on his second day on site. The HSE and Training Manager wants the investigation done using the incident cause analysis method and the Incident Investigation Report Template, with the draft report and actions within seven days.

Your brief. Lead the investigation of the ramp 4 incident: gather the facts and records, interview both drivers, build the timeline, identify the failed controls and their organisational causes, write the report on the template and enter the corrective actions in the Hazard and Incident Register.

What to look for

Evidence guide

The report must separate facts from opinion, show the timeline built from evidence, and trace the missed call to organisational causes such as contractor induction, road signage, radio channel allocation or supervision rather than stopping at the driver. Look for a potential consequence rated as fatality and actions that change the road or the access rules, not only retraining.
